Description: Social Service Tech 3/ Vocational Specialist - Assertive Community Treatment (ACT) 8-430 M-F On Call
- Provides vocational assessment to consumers who are interested in work and educate them on the role of the vocational technician on the team.
- Responsible for teaching vocational and pre- vocational skills such as time management, resume building, job seeking, interviewing, and on the job support.
- Maintains a caseload of 10-12 consumers as assigned, acting as the main point of contact for those consumers, ensuring a scheduled MD appt that month, and assisting in arranging transportation/ planning.
- At least 80% of services provided to consumers is in the community. Attends average of 4 weekly treatment team meetings.
- Available for after - hours (crisis phone) coverage as assigned.
